would like to welcome you to our office.

To make your visit to our office more expedient, please click here to find your Patient Registration Form and a Medical History Questionnaire. Please complete these forms prior to your visit and bring them with you on your appointment day.

Bring any medications and eye drops you are currently using, along with your eyeglasses and/or contact lenses.

First visits can take up to two hours. You should expect that your eyes will be dilated.

Also, have all of your insurance cards and a current credit card with you. If you have insurance coverage that requires pre-authorization or a referral form, please obtain this prior to your visit to the office and bring it with you.  Applicable co-payments and deductibles must be collected at the time of your visit. Payment for any services that are not covered by your insurance must also be paid at that time. Payment may be made in cash or by check (under $200) or credit card (Visa, MasterCard, and Discover).

 

Please arrive at least 15 minutes prior to your appointment, and please bring the following items on the day of your visit:

  • Photo ID (i.e. Drivers License)
  • Insurance card(s)
  • New Patient Form

Late Notice:

If you anticipate being more than 15 minutes late to your appointment, please call to notify the office. We will make every effort to accommodate you, but to be fair to all patients, we may have to reschedule your appointment.
